\"\"Pacjent w celu uzyskania dofinansowania z NFZ, nie musi ju\u017c udawa\u0107 si\u0119 osobi\u015bcie do osoby uprawnionej (lekarza, felczera, piel\u0119gniarki, po\u0142o\u017cnej, fizjoterapeuty) po zlecenie na zaopatrzenie. Wystawienie takiego zlecenia mo\u017ce\u00a0 by\u0107 dokonane w ramach „teleporady”.<\/p>\n

Pacjent, po pozytywnej weryfikacji zlecenia przez NFZ, otrzyma od wystawiaj\u0105cego zlecenie\u00a0\u00a0\u00a0<\/span><\/b>informacj\u0119 o wygenerowanym przez system e-ZWM numerze zlecenia i informacja ta r\u00f3wnie\u017c mo\u017ce by\u0107 przekazana\u00a0 (telefonicznie, e-mailem lub SMS-em).<\/div>\n

W sytuacji, gdy zlecenie na zaopatrzenie w wyroby medyczne zostanie wystawione „tradycyjnie” w formie papierowej poza systemem e-ZWM, wymagane jest nadal jego potwierdzenie w Oddziale Wojew\u00f3dzkim NFZ, ale zlecenie to mo\u017cna przekaza\u0107 do dowolnie wybranego OW NFZ z wykorzystaniem system\u00f3w teleinformatycznych lub innych system\u00f3w \u0142\u0105czno\u015bci drog\u0105 elektroniczn\u0105,\u00a0<\/span><\/b>np. skanem, mailem lub faxem.<\/p>\n

Realizacja zlecenia w sklepie medycznym\u00a0 odbywa si\u0119 po podaniu numeru zlecenia i numeru PESEL pacjenta, a w przypadku os\u00f3b bez numeru PESEL daty urodzenia – (sytuacja bez wydruku papierowego). Pacjent podpisuje jedynie dokument potwierdzenia odbioru wyrobu medycznego.<\/p>\n

Dopuszczalne jest niepotwierdzenie odbioru przez pacjenta (potwierdzenia dostawy dokonuje osoba dostarczaj\u0105ca przesy\u0142k\u0119, a apteka lub sklep ortopedyczny, zachowuje dokument potwierdzaj\u0105cy dostaw\u0119).<\/p>\n
